Agreement to Provide Supervision

After Hours Student Enrichment

I the undersigned agree to assist the classroom teacher, provide tutoring, homework help, or coordinate activities to the best of my ability for the students enrolled in After Hours Student Enrichment. I may work from release to 6pm on selected days, or for a mutually agreed on time frame on selected days as needed. I will honor my time commitments, and our students, and behave in accordance with our governing school guidelines, and our school behavior expectations. I understand I may be earning monetary compensation for my time, at a rate of $13-$17/hour. I may be working from 1 day per week up to 4 days per week, and I may fill out a W-4 form and taxes may be withheld from my wages. I will be provided a W-2 at the end of the calendar year prior to Jan 31st of the following year. I am responsible to file appropriate tax returns as required by the IRS.

Any conflict or disagreement I may have with any student or staff affiliated with After Hours may be brought to the attention of Student Enrichment Services program director. I am not to discipline any student participating in the program in any way. I further understand I am not to be alone with any student under any circumstances, but am to remain in the designated space (classroom with door open) with all the students, "private" conversation may occur in the hallway outside of our classroom. Students may be taken to the restroom in small groups of 2-7 students and may be supervised from the hallway only.

The purpose of my involvement is to create a safe and enjoyable environment, provide homework help, or tutoring, and encourage fun games and activities for LPS students staying after normal school hours.
